RESUMO

The aim of the study was to design an accurate and useful observational instrument for evaluating basic turn techniques in alpine skiing, and to determine its validity and reliability. A multi-step approach was used to design and validate the instrument: a literature revision; an expert meeting; a pilot test; the assessment of intra- and inter-observer reliability; a convergent validity test; and the responsiveness calculation with a pre- post-test. Results showed adequate values of intra-rater (P=0.80-0.83) and inter-rater reliability (P=0.86-0.97). For the convergent validity, a moderate statistically significant correlation for the speed and trajectory control dimensions were achieved (rho=0.58-0.65, p<0.001), but not for the balance position dimension (rho=0.24, p=0.166). The responsiveness was also acceptable (p<0.05, r=0.46-0.60). In conclusion, the Alpine Ski Technique Observation Instrument is a useful, reliable, valid, and responsive observational tool. (AU)


El objetivo del estudio fue diseñar un instrumento de observación preciso y útil para evaluar la técnica de cambios de dirección básicos en el esquí alpino, y determinar su validez y fiabilidad. Para diseñar y validar el instrumento se siguieron los siguientes pasos: revisión de la literatura, valoración por comité de expertos, prueba piloto, evaluación de la fiabilidad intra e inter-evaluador, prueba de validez convergente, y cálculo de sensibilidad con una prueba pre y post-intervención. Los resultados mostraron valores adecuados de fiabilidad intra-evaluador (P=0,80-0,83) e inter-evaluador (P=0,86-0,97). Respecto a la validez convergente se obtuvo una correlación moderada para las dimensiones de control de la velocidad y de la trayectoria (rho=0,58-0,65, p<0,001), pero no para la dimensión de posición de equilibrio (rho=0,24, p=0,166). La sensibilidad fue aceptable (p<0,05, r=0,46-0,60). En conclusión, el Instrumento de Observación de la Técnica en Esquí Alpino es una herramienta útil, fiable, válida y sensible. (AU)

Existe escasa literatura sobre las asimetrías en el pádel, un deporte de naturaleza asimétrica. Para estudiar el desarrollo de asimetrías en este deporte se evaluó, con bioimpedancia, la masa magra de ambos miembros superiores de 96 jugadores de pádel jóvenes y de 76 esquiadores (grupo control) y se calculó el índice de simetría. Los jugadores de pádel tuvieron una asimetría entre ambos miembros superiores mayor que los esquiadores, cuando se consideró toda la muestra (7.2 ± 5 % vs. 1.4 ± 3.2 %; p < 0.001), en jugadores con un estado de madurez negativo (5,7 ± 3,2 % vs. 1,5 ± 3,8 %; p < 0,001) y en aquellos con un estado de madurez positivo (8,3 ± 5,8 % vs. 1,3 ± 2,4 %; p < 0,001). Este estudio revela que el pádel genera asimetrías de masa magra en los miembros superiores, incluso antes de la edad de crecimiento pico. (AU)


There is little literature on asymmetries in paddle tennis, a sport of an asymmetrical nature. To study the development of upper limb asymmetries, 96 young paddle players and 76 skiers (control group) were evaluated via bioimpedance. The lean mass symmetry index was then compared, considering the sport (paddle tennis players and skiers) and the maturity offset (positive or negative). Paddle-tennis players had a systematically greater upper limbs asymmetry than skiers (7.2 ± 5 % vs. 1.4 ± 3.2 %; p < 0.001). This also occurs when comparing only the subsamples with a negative maturity offset (5.7 ± 3.2 % vs. 1.5 ± 3.8 %; p < 0.001) or with a positive maturity offset (8.3 ± 5.8 % vs. 1.3 ± 2.4 %; p < 0.001). The study reveals that paddle tennis generates asymmetries of lean mass in upper limbs, even before growth spurt. (AU)

Whole-body vibration (WBV) is being promoted as an efficient complement to resistance training. The aim of this study was to investigate the effects of an 8-week program of WBV in combination with resistance training on knee extensors muscular performance. A group of 29 young adults (25 men, 4 women; age 21.8 ± 1.5) performed a WBV plus resistance training program (WBV + RES) or an identical exercise program in absence of vibration (placebo plus resistance training, PL + RES). Participants were evaluated for anthropometry, muscle strength (half-squat three repetition maximum, 3RM), knee extensors isokinetic dynamometry (180° and 60° s(-1)) and counter-movement jump (CMJ). After the intervention, percent body fat significantly decreased 2.1% only in WBV + RES (P < 0.001), while muscle mass significantly increased in both groups (P < 0.01): 2.2 and 2.8 kg in PL + RES and WBV + RES, respectively. No significant differences were observed in isokinetic strength or CMJ, and 3RM significantly increased in both groups (P < 0.001): 64.2 kg (52% of baseline) in PL + RES, and 46.9 kg (43%) in WBV + RES. The addition of WBV to resistance training during 8 weeks, in recreationally active young adults, did not result in a larger muscular performance improvement compared to an identical exercise program in absence of vibration. Muscle mass also seemed to be equally affected with or without vibration, yet body fat could be exclusively decreased by WBV. Further research is required to clarify whether WBV, as a complement to resistance training, produces additional specific benefits.

INTRODUCTION: In patients operated for herniated lumbar disc it could be very useful to develop physical examination criteria that would allow early prediction of medium or long term clinical outcomes. The result of the Lasegue test after surgery depends on the occurrence of recurrent herniated disc and/or scar formation around the nerve root. Previous studies have shown the association between the result of Lasegue test, assessed 4 months after surgery, and the short term functional outcome. OBJECTIVE: To determine the prognostic value of postoperative Lasegue test in relation with medium term functional and clinical outcomes which were estimated by analyzing the functional grade, employment status, quality of life, reoperation for recurrent disc herniation, and the number of clinical revisions and imaging studies needed during the follow-up. PATIENTS AND METHODS: 243 patients who underwent hemilaminectomy for lumbar herniated disc between 1996 and 2002 were studied. The Lasegue sign was tested 3 months after surgery. A modification of Spangfort's criteria (to measure the functional grade) and the 12-item Short-form health survey (SF-12) method (to measure the quality of life) were used to quantify outcomes at 12, 24 and 36 months after initial surgery. RESULTS: This investigation has proved the prognostic value of Lasegue test assessed 3 months after initial surgery. A statistically significant correlation was found between a positive straight leg raising test and a poor functional status at 24 and 36 months, and the risk do not return to work because of persistent pain at 12, 24 and 36 months. A statistically significant correlation was also found between the result of the test and the frequency of reoperation for recurrent disc herniation and the number of clinical revisions and imaging control studies needed through the follow-up. Likewise, the prognostic value of Lasegue test on different aspects of quality of life (health perception, functional performances, poor social interaction and severe pain) analyzed at 24 and 36 months after surgery were also confirmed. CONCLUSION: This study provides additional information about the predictive value of the straight leg raising tested 3 months after surgery on the clinical (diagnostic resources consumption, successive outpatient revisions, etc) and functional (quality of life, functional grade and return to work) outcomes assessed 24 and 36 months after initial surgery.

RESUMO

The hormone sensitivity of some tumors seems to be mediated by the presence of specific receptor proteins, and a correlation seems to exist between the amount of receptor molecules and the behavior of the tumor evolution. Epidemiological data suggest a relation between the steroid sexual hormones and the development of some tumors of the central nervous system (CNS). The authors determine the amount of receptors specific to 17-beta-estradiol and progesterone in several cases of meningioma, glioma, neurinoma and intracerebral metastases. 17-beta-estradiol receptors were always detected, although in very variable amount (3 to 74 fm/mg protein). Progesterone receptors were found in all the studied CNS in women, and only in a few male gliomas, in amounts varying between 3 and 17 fm/mg protein. The significance of hormone receptors in the CNS tumors need further studies to know if they can be applied to prognosis and suggest the assay of a complementary endocrine therapy of CNS tumors.
